Your First Visit
Your first consultation will involve an examination, as well as a diagnosis. From there, we will explore your options for any further treatments you may need. If necessary, our staff will work with you to set up additional appointments. For a speedy check-in at your first appointment with us, please bring your doctor referral (if applicable), any past x-rays, a list of any medications you are currently taking, a completed Health History Form, and don’t forget your insurance card and ID!
Special Circumstances
If you have any special medical conditions, please inform your doctor at the start of your visit. Also please note that a parent or guardian should be present at the consultation if a patient is under the age of 18.
